Bankruptcy Debtor Claim Form H For Scheduled Debts

This form must include all debts owed, regardless of nature and disputed status. Inclusion of debts within the debtor's claim form, schedule H, is the record upon which discharge will be granted. Any debt omitted from this schedule will not be discharged, so it is in the best interest of all parties that this list be as complete and compressive as possible. Discharge does not affect any debt that is omitted, and potentially, if an omission was intentional, the court may dismiss the case.
